what is a care Co ordinator?

Anyone know what a care Co ordinator is I've just been appointed one and feel very scared what does she expect/want from me?

Hi, care coordinators are meant to support you, to make sure that you are getting the right help, liaising with other staff who support you. When you meet she should explain her role and ask what you need. They are usually nurses, occupational therapists, or social workers who work in the mental health team. She should heIp devise a care plan with you to help you.
I see mine weekly, current one is very good, I've had mixed experiences. She listens to me, makes referrals as needed, liaises with psych, cpn and GP. etc.
