People discuss anything they want. Some people who go are comfortable opening up the talk, some not. I would ask the therapist to lead if I were you. Tell them you prefer them to ask questions if that helps.

Struggles, some people need someone who help them to solve their issues.

Others use it as a relief, just having someone to talk.

I talked to a social worker a few years ago when I was not well. I think it helped. I was quite talkative at the time, but my mental state was such a mess. I'd feel fine while talking to her, but otherwise was going through all sorts of inner oddness. I wouldn't mind having someone to talk with these days. I live with two brothers but our communications are either basically nonexistent or really unsatisfying to me.

I think the main thing is to connect with whoever you're talking with. If you're comfortable opening up about things it can be good to have someone with a sympathetic ear. The social worker said I was really open about things, but throughout my life I've always been quiet with people. I suppose the illness made me talkative then.

Therapy requires trust and mutual respect between client and therapist. That's not something easily established. It took many sessions before I felt I could actually open up, be vulnerable and discuss things of actual importance about my life.
Here's what I mean by trust and respect:

I trust that my therapist won't judge or shame me for anything I reveal. I trust that what is said will remain confidential. My therapist trusts that I won't lie about or omit important things about my life, that I want to be helped and I am committed to our established goals of treatment.

I respect my therapist's credentials, expertise and clinical experience and she respects my autonomy, beliefs and cultural context.

You might ask yourself, what would I open up about-- if I completely trusted and respected my therapist? I couldn't possibly tell anyone that! Yes, that is precisely what you could and would talk about.

I hope this is useful. Hoping you find the right therpist who "clicks" with you.

My therapist is on Skype so I can sit at home with a cup of tea. We just chat about every day happenings and if she spots something we can work on she stops me. I only need her about once a month now as she has done such a good job. You need to trust the person you are speaking to.
