How do your get your Doctors out of the loop of "CBT/Anti Depressents"

Blah blah 15 years depression, anxiety etc.

No matter what I say to them it's the same old line every time, "have you tried this anti-depressant before... we'll start you on a low dosage...".

I know there is a way to get a diagnosis or at least someone to better evaluate what's going on but the question is how. I have tried being calm and reflective, putting the best foot forward, having a meltdown, telling them that I am thinking about hurting people (all on separate occasions, of course, just listing the approaches) and it all gets the same result.

I was thinking about smacking my Doctor in the mouth but he ended up being off sick and I got a lady instead so that plan was out.

What can you do to get noticed?

I've thought about suicide attempts but the fear there is that it would just be the same as everything else. On the plus side, I am employed... what an inspiration ;)

i would ask to be referred to a psychiatrist, if for some reason they wont seem to do this for you i would then find out the local mental health team in your area and contact them directly

I don't know but your symptoms might be within a range of ordinary clinical depression.Contrary to some beliefs, common depression may worsen personality, improper aggressive thoughts and behabiour may occur, not just "low mood".Right antidepresant or other antidepr. therapy may make a lot of differtence.Try that first and see what next.
Don't be in such a hurry to get tough psychiatric treatment.In fact you seem to be extremely lucky if from what I gather you can get antidepressant prescription from ordinary doctor and don't have to go to a local "mental team" for that ( and maybe ruin your future environmental, job, marital and other prospects).
