People can guess if your behaviour is strange. Do you have any professional that you can open up to? Anyone to confide in?

Wearing baggy clothes. Refusing foods you would usually eat, claiming to have already ate everytime food is offered.

It depends on the individual and how they hide their eating habits, and it also depends on how in tune friends and family are to be able to notice the subtle signs.

I dont wear baggy clothes, but I tend to refuse things I'd usually eat when I'm going through a bad spell.

They could notice weight loss, change in mood (like depression), hair thinning, change in complexion, dizziness..or behaviors like coming up with excuses not to eat, avoiding social situations where food is involved, dietary restrictions, picking at food, wearing oversized clothing...

Usually people start to notice when your weight drops or when you avoid eating or eat very little.

People with me notice when I avoid eating in company,
I may Cook for others or arrive at a friends at meal times,
I will answer (whatever is appropriate at the time) ..I had a late breakfast, I've only just had lunch, ..I'm have leftovers at home that need to be eaten.
If I get a sudden invite for tea I will reply late that ..' I only just got yr text, sorry..we can catch up another day.
I don't have anorexia nervosa but I do have a restrictive eating issue going on
People will notice that I have lost weight and may ask if I'm OK, they aren't silly no matter I can't fool them
