help with antidepressants with bpd

i was recently told i have bpd, having been told by my therapist in the past i was a "textbook case" of bpd. a while ago my psychiatrist and i decided to put me on antidepressants, mainly due to my depression because i'm not responding well to therapy and it tends to make me worse. at first i was put on sertraline (an SSRI). i was on this for 2 months, with 2 dose increases, and each time i had my dose increased i would have a couple of weeks of terrible instability. it made my borderline episodes 10 times worse. it also made my sleep worse, and it made me dissociated all the time. it also made me highly nauseous and i even threw up once, so i was taken off it. after that i was put on mirtazapine, an NASSA, because it was obvious that i was unlikely to respond well to other SSRIs. and i've been on mirtazapine for 5 weeks now, and so far it's done shit all as well. the only thing it's done is help me with my sleep but that's a side effect which i've had since day 1. i'm always tired and it hasn't done anything to help with my depression or mood or anything. all it's done is made me more hungry, but i also struggle with disordered eating and calories counting, and this has made it 10 times worse. it's obvious that this dose of mirtazapine isn't helping me but i'm afraid if i go up my eating disorder will get worse. the only other drug classes i have left to try are SNRIs such as venlafaxine (but because of the similarity to SSRIs that's unlikely to help), or TCAs/MAOIs, but those are more deadly in overdose and i'm a high suicide risk so it's unlikely i'd be given those.

i was also told that borderlines don't respond well to inpatient treatment, so i guess i'm just a little lost... does anyone else have any suggestions for meds that helped them? everything feels like it's always getting worse all the time and my favourite person tries so hard to help me but he doesn't have bpd and no matter what he says i know he's always thinking less of me for the things i get set off by.

Just because one SSRI caused side effects doesn't mean they all will. I would ask to try a different one. There are many

Citalopram
Escitalopram
Fluoxetine

Mirtazapine made me sleep and gain weight but it did get rid of my depression within 2-3 weeks.

Tp become more unstable on anti depressants could be a sign of bipolar disorder, but i don't know very much about BPD.

I’ve tried them all and none of them helped me significantly or in some cases did for a bit then stopped some made me actively suicidal and mirtazapine worked for a bit but in the end I had to stop because I was passing out in the shower then unable to get up I also put on like 9kg in like 6 weeks isn’t good and it gets embarrassing order dominos two for Tuesday twice in one night when they know you live alone 😳🙊 but I know for some people they’ve worked miracles!

also I think the inpatient thing is a cop out, they think people with bpd are treatment resistant I think antidepressants would help feelings of chronic emptiness and boredom that we feel but for other symptoms like the emotional rollercoaster etc you need to try and get DBT therapy google it hopefully that helps a bit x

I’m sure you’re favourite person doesn’t think less of you it’s just such an intense thing to experience for them and they want to help but they’ll never get to experience what you experience so it’s hard ...which is frustrating for you but they’re your friend and they love you remember that and try and be kind coz we have a tendency to scare people away sometimes when we’re super emotional x

Have you tried a mood stabilizer with your antidepressant?

Hi i have had fluoxetine and it didn't agree with me gave me the shakes but i have trazadone at night and that has some what helped a little just because i have very disturbed sleep with out it drs say i have anxiety and panic attacks while sleeping almost like night terrors but i don't recall dreaming.

Many SSRIs were bad for me but now I take venlafaxine SNRI and it's okay, also mood stabilizers, and few days ago I got xannax for anxiety attacks

I'm not borderline so I can't comment from personal experience. However, from what I do know there is not a single medication that can help with the core symptoms of Borderline Personality Disorder. Antidepressants can help with associated depression and anxiety, but do not treat the condition itself. Some mood stabilisers, such as Lamotrgine and Lithium have shown promise in reducing mood swings and impulsivity, but they are primariliy Bipolar drugs and in the UK they tend to not prescribe things off label. That being said, if it gets bad, ask to be referred to a specialist psychiatrist, not therapist or GP, and they may be able to help with this.

But the mainstay of BPD treatment is therapy, especially DBT. The only proven cure to reducing core symptoms and improving functioning. So engage with it.

I have two antidepressants and also quetiapine. This really stabilized me when I was out of control with my BPD :)

I have found some therapy helpful. Basically it came down to finding a therapist I could engage with.

Don't lose hope! it often takes time to find the right combo. You can try SNRI if your doc wants to try. There is also the opertunity to combine an antidepressant with a mood stabiliziser.

If you have to take a medication that can rise an urge to suicide, please have the number for an emergancy call visable at a table and remeber to use it if so. ☺

The doctors in the uk just don't cope well with mental health. Infact. They dont cope with anything if im honest. As for DPT ive only found out about it recently. I went to see a psychiatrist and had no help at all. Other then here are some you tube recordings to watch. If you need to come back again you can be referred by your gp. A load of rubbish. Really struggled to get help. Im lucky I have a supportive partner. Or my hell would be so so different.

Sadly it is a case of trial and error in finding the right meds for you. I take sertraline with low dose olanzapine as a mood stabiliser - it’s the best combo for me, not perfect but better than anything I’ve tried before.

Im on the highest dose of sertraline 200 mg. Have been for over a year and a half. No follow up nothing not a thing. So im lowering my dose according to advice from NHS. I have suffered all my life one way or another. Being taken advantage of etc. By the very people that was meant to protect me.
I've been told I'm resilient I can get through anything. And they are right. And these tablets make me a zombie. I haven't been given mood suppressants.
My doctor hadnt even checked my notes to see that I had a mental health history. No one cares. So you have to care. And keep fighting. Even if im exhausted every goddamned day. I can only manage one 12 hour shift a week. And need time outs. But im alive.
