I don't know what is causing these:
  1. Inability to concentrate
  2. Inability to understand time or know when something happened
  3. Blanking out
Any replies will be appreciated!

That sounds like dissociation to me.

What usually causes dissociation is stress. It is our brains way of protecting us from something we find traumatic. I do not know if it is caused by a particular condition. I feel it is more the brains response to protect us from the pain.

Oh wow. It could be.
Could anxiety or bipolar cause dissociation?
Every person on the planet dissociates, it's not a disorder unless it severely impacts your life in a negative way.

Well, yeah. I can't seem to get anything done because of it. It's probably just stress though.

Have you tried grounding techniques to help lessen the dissociation? That can help immensely

I find an ice cube works really well. The shock of the cold makes me more alert.
