Difference between community MH and primary care MH?

Im slightly confused to the difference between the two. Or are they the same thing. Asking from the UK

Primary care is usually your gp, as they're the first person most people will see about a MH issue.

Secondary care is usually the Community Mental Health Team, which is generally psychiatrists / social workers / CPN / etc (if you need them).

Generally a gp will refer you on to the Community team if they think you need it, otherwise you'll stay with primary care (GP).

Thanks for the response. Yeah i was referred to CMHT a while back but they weren’t the best and just discharged me. Went back to GP because i still needed help and she reffered me to primary care mental health team.

I know that some GP's surgeries have a mental health nurse attached to the team ,if that's what you mean?
