Abstract
BACKGROUND:

Hormonal deregulation is associated with suicidal risk in various psychiatric disorders. Thyroid hormones and prolactin play a role in the pathophysiology of schizophrenia. The present study aimed to analyze thyroid hormones and prolactin levels in patients with schizophrenia, and to explore its association with disease severity and suicide risk.
METHODS:

Thirty-eight cases and 38 controls were included in the study. Serum thyroid hormones and prolactin were estimated in all the subjects. Disease severity was assessed using Positive and Negative Syndrome Scale and suicidal ideations using Columbia Suicide Severity Rating Scale.
RESULTS:

Serum prolactin (p=0.004) and free T4 (p=0.029) were significantly elevated in the schizophrenia group as compared to controls. Serum prolactin was significantly associated with higher negative symptoms scores (r=0.418, p=0.008), but not positive symptoms or general psychopathology of schizophrenia. Thyroid hormones were not associated with disease severity scores. Suicidal ideas were more common in schizophrenia patients higher free T4 (p=0.011).
CONCLUSION:

Prolactin and free T4 were increased in patients with schizophrenia as compared to controls. Suicidal ideation was associated with increased free T4, but not prolactin levels.

If prolactin levels are associated with negative symptoms I wonder how this relates to risperdal/risperidone and the fact it can increase prolactin levels. Is this a reason to suspect increased lack of drive and motivation while on it can be medication related?

It is not clear from the Abstract whether the people with schizophrenia were on medication. Several antipsychotics raise prolactin levels and some drugs affect the thyroid. How can they tell apart the effects of the illness against the effects of medication?

I am presuming drug free as mentioned in the title meant not on medication. Even if medication related if some antipsychotics raise prolactin and prolactin levels affect negative symptoms that presents a problem.

This research though seems to point to increased prolactin in drug naive/free people with schizophrenia.

Abstract
OBJECTIVES:

The present study was to examine the relationship between serum levels of prolactin and the inflammatory status in drug-naïve, first-episode schizophrenia patients with normal weight.
METHODS:

Patients with normal weight, drug-naïve, first-episode schizophrenia and healthy controls were enrolled in the study. Serum levels of prolactin (PRL) were measured using electrical chemiluminescence immunoassay. Serum levels of interleukin-1β (IL-1β), tumour necrosis factor-α (TNF-α) and interleukin-6 (IL-6) were examined using enzyme-linked immunosorbent assay (ELISA).
RESULTS:

Sixty patients with normal weight, drug-naïve, first-episode schizophrenia and 60 healthy controls were enrolled. The schizophrenia group had higher serum levels of PRL, IL-1β, IL-6 and TNF-α compared with the control group. There was a gender difference of hyperprolactinemia in schizophrenia group. There were positive relationships between serum levels of PRL and serum levels of IL-1β, IL-6 and TNF-α within the schizophrenia group. Within the schizophrenia group, TNF-α was the strongest predictor among the three cytokines for serum levels of prolactin after controlling for gender, age, education, smoking status and disease duration.
CONCLUSIONS:

Patients with normal weight, drug-naïve, first-episode schizophrenia present elevated serum levels of PRL, which might be related to the up-regulated inflammatory status in this patient population.

(Sorry, wasn't paying enough attention).

That is very interesting. Especially the link to inflammation. So many health problems seem to be related to that - or maybe the inflammation is a response to the health problem or the body's response to it. Just guessing not very knowledgeable about this.
